I am an in-network provider for UnitedHealthcare, Oscar, Optum, Aetna, Cigna, BlueCross/BlueShield, and Oxford plans.

 

I also accept out-of-network benefits if I am not in-network with your plan. I can provide you with a monthly invoice to submit directly to your insurance company for reimbursement. I recommend that you call your insurance company to ask about your out-of-network benefits before we meet.

 

I offer sliding scale rates on a case-by-case basis.
